Fillers & Volume Restoration

The development of new filler substances represents one of the most exciting areas in cosmetic surgery, as these new fillers promise to last longer than prior generations of fillers while greatly reducing the risk of allergic reaction. New filler substances available include Restylane®, Juvederm™ Ultra, Radiesse®, Sculptra®, Evolence®, Cosmoderm® and Cosmoplast®. These fillers are injected under facial creases and lines to restore fullness and erase deep set lines. Areas that are commonly treated include the lips, nasolabial folds, vertical lip lines and other facial wrinkles. Fillers also work well for hand rejuvenation, where they can restore the youthful fullness of the hands and help hide the veins and tendons that stick out with age. A few simple injections can dramatically revitalize your appearance without the need for surgery.

For the more persistent or deeply embedded facial lines, furrows or creases, a filler is the best option. Fillers are also great for volume augmentation of the cheeks and under-eye hollows, to restore the fullness of youth.

When receiving filler injections, some patients also opt to receive Botox injections to their face, crow's feet and frown lines. Used in combination with fillers, Botox satisfies the growing demand for effective, safe, and no downtime cosmetic treatment. And that suits Dr. Ort just fine, as he is a conservative dermatologist and never urges any client to move faster toward a decision about treatment than that client wants to move.

Trouble Area Photographic Example Caused by Result
Nasolabial Folds (the lines that extend from your nose to the corner of your mouth) Smiling and descent of malar fat pad from cheek Deep "parentheses" lines that distract from the face and give you a gaunt or jowling appearance
Marionette lines and jowls Descent of buccal fat pad, muscle activity Downturned smile at corners of mouth, jowling along jawline
Lip lines in the area between your nose and lips Smoking, puckering and general facials expressions An aged lip area

The amount of filler used is always adjustable, giving the doctor the ability to control your results. Fillers also work great to restore the fullness and suppleness in your lips.

Restylane® & Perlane®

This long-lasting filler is made of hyaluronic acid, a naturally occurring substance in the body that is injected into the skin in small amounts by an ultrafine needle. Modified slightly to last longer than naturally occurring hyaluronic acid produced by the body, its sister product is Perlane. Hyaluronic acid attracts and binds water, which is what gives your skin its turgor and fullness.

Perlane is the sister injectable to Restylane, a hyaluronic acid produced by the body naturally. While Restylane lasts up to six months or longer, the particles in Perlane are bigger and can last longer than six months. Perlane is designed for deeper injections and folds than Restylane, such as the folds around the mouth, sometimes called marionette lines.

Over time, Restylane and Perlane are naturally and safely absorbed by the body.

Juvederm™ Ultra and Juvederm™ Ultra Plus

Juvederm injectable gel replaces the hyaluronic acid that is lost over time as we age, resulting in facial wrinkles and folds. Injected into the dermis, it smooths away wrinkles and folds and makes an immediate difference, restoring your face to a much younger-looking you. The procedure requires no downtime and it works on all skin types, plus is approved to last up to a year.

Radiesse®

Radiesse is a next-generation dermal filler that not only fills, but volumizes and works with your body to stimulate new collagen production. This safe, FDA-approved dermal filler is injected below the skin's surface and restores a natural, long lasting, youthful look. Made of unique calcium-based microspheres suspended in a natural gel that is injected into the skin through a simple, safe and minimally invasive procedure, Radiesse works with your body to form support "scaffolding" that stimulates the growth of your own collagen. Over time, the calcium-based microspheres gradually break down and are safely and naturally absorbed by your body.

The results of Radiesse are immediately evident and may last a year or more in many patients.

Sculptra®

Sculptra® is a unique filler which is injected over the course of several sessions. By stimulating your body to produce its own collagen, Sculptra gives a natural filling and volumizing effect. Sculptra is used to restore volume throughout the face, such as in the cheeks, under the eyes, temples, and around the mouth.

Collagen

Cosmoderm and Cosmoplast are human collagen fillers. Unlike the old bovine collagen which could cause allergic reactions and required skin testing, these fillers have a much lower risk of allergic reaction. Cosmoderm is a thin filler which is particularly suited for fine lines such as the vertical lines on the upper lip. Evolence® is a new collagen filler which promises to last longer than traditional collagen fillers.
